Personal AssistanceWe support and provide direct care assistance with activities of daily living to people who choose to continue to live with their families at home. Our well trained staff create opportunities of learning while providing direct assistance with activities of daily living such bathing, dressing, personal hygiene, eating, meal preparation, household chores, budget management, medication administration and attending appointments. TPA provides this service in the home or in the community closely following the plan of care/ISP. When Individual transportation to and from the community is approved for the person supported, TPA will provide it in accordance with the ISP and DIDD requirements.

Day ServicesOur Day Service enables people supported to participate in meaningful community activities that provide opportunities to acquire, retain or improve skills. The skills learned are necessary to perform or enjoy basic activities such as shopping, banking, employment, volunteering, visiting parks, recreational facilities, or libraries etc. Three options of Day Services are available. These options can also be provided as a combination or separate service: Community Based Day Service: This option is provided outside the home of the person supported in a community setting. The service provides opportunities to explore, discover, participate, and enjoy meaningful community activities. Transportation is provided to access community facilities and resources. A minimum of six hours of service is provided Monday through Friday in the community. In-Home Based Day Service: This option is provided in the home of the person supported and it is mostly available to people that have chosen to retire or those that cannot safely participate in community activities due to behavior or medical reasons. We provide opportunities to learn basic home skills and engage in meaningful activities whilst at home. A minimum of six hours of this service is provided Monday through Friday at home. This service can also be combined with Community Participation services and the person supported can choose to use either one of the services on a different day. Supported Employment: This option allows people supported to acquire job skills and to be gainfully employed in their local communities. TPA has qualified Job Coaches who provide ongoing assistance and specialized supervision with job skills/duties as needed to enable the person supported to sustain employment A combination of both supported employment and Community Participation services. TPA provides reliable transportation that accommodate the needs of each person supported.

Supported LivingOur Supported Living program assist people to choose and live in a house or apartment in a community of their choice with a roommate or companion of their choosing. Direct personal support is provided by well trained professional staff in a manner that promotes independence and decision making. Our well trained staff create opportunities of learning when providing direct assistance as needed with activities of daily living such as bathing, dressing, personal hygiene, eating, meal preparation, medication administration and household chores essential to the health and safety of the people supported. Our individualized supported living supports will enable a people supported to acquire, retain, or improve skills necessary to reside in a home and enjoy a safe and a meaningful life. Upon request and where the Person Supported or Conservator cannot manage financial affairs, TPA provides Rep Payee services in accordance to Social Security Administration and DIDD's personal funds management policies and procedures. TPA provides supported living services alongside transportation services in a manner that meets the health, safety and social needs of the person supported. Program Highlights: People Supported choose where they want to live. People choose their own roommate(s). People are supported to self direct their services. People are supported to make personal decisions and shape their destiny. People are supported to pursue their dreams. People are supported to manage their own finances.

Medical Residential Supported LivingOur Medical Residential Supported living program supports people who need direct skilled nursing and habilitative services so that they can continue to live in the community without being institutionalized or hospitalized. TPA provides well trained nurses who provide skilled nursing services on a daily basis as specified by the person supported 's plan of care. Our direct professional staff will assist with activities of daily living (e.g., bathing, dressing, personal hygiene, eating, meal preparation), household chores, budget management, attending appointments and interpersonal and social skills building to promote independence. Upon request and where the person supported or conservator cannot manage financial affairs of the person supported, TPA provides Rep Payee services in accordance with Social Security Administration and DID's personal funds management policies and procedures if appointed to do so by the person supported and or conservator. TPA provides medical residential supported living services alongside transportation services in a manner that meets the health, safety and social needs of the people supported. Our Family Model Residential Support (FMRS) is a type of residential service where a person supported chooses to join and live in the home of a trained caregiver who is a not family member. The person supported is integrated with the new family and becomes a member of the family model provider(caregiver)'s household. Under this arrangement, the person supported receives individualized, person-centered services and supports that promote the person supported to acquire, retain, or improve skills necessary to reside successfully in a family environment different from the family of origin. The service includes direct assistance as needed with activities of daily living (e.g., bathing, dressing, personal hygiene, eating, and meal preparation excluding cost of food), household chores, budget management, attending appointments, and interpersonal and social skills building. It also includes medication administration. TPA will recruit, screen, train and supervise the Family Model Residential Support provider/caregiver. Our family model program does not permit family model residential homes to support more than 3 residents regardless of the funding source. RespiteOur respite program provides temporary relief staff for a number of days or hours to allow families or caregivers some time to rest or take a vacation. The respite personal assistant assists with activities daily living such as bathing, dressing, personal hygiene, eating, meal preparation, household chores, budget management, medication administration and attending medical appointments. TPA provides this service in the home or in the community closely following the plan of care/ISP. Individual transportation to and from the community in accordance with the Waiver definition is also provided.

NursingWe provide licensed nurses (LPNs and RNs) to help care for people supported who are medically fragile and need skilled nursing services. We provide this service in home and community settings only. We cannot provide nursing in inpatient hospitals, nursing facilities and Intermediate Care Facilities (ICF). Our well-trained Nurses provide skilled nursing care with dignity in a manner that improves the quality of life; following the treatment plan specified in the plan of care. We provide skilled nursing services within the scope of Tennessee’s Nurse Practice Act. Before we can commence services, the Nursing Services must be ordered by the person supported’s physician, physician assistant or nurse practitioner, who shall document the medical necessity of the services and specify the nature and frequency of the nursing services. The nursing plan of care shall be included in the ISP in accordance with DIDD requirements.

Senior ServicesOur Senior or Elderly program provides home and community based services that enable elderly and people with disabilities to live independently in their homes or in integrated community living settings as an alternative to nursing home placement. Our support services are delivered with well trained staff in a manner that allows senior people to continue to live an enjoyable, successful and meaningful life in their homes or community based residential homes. Supports include bathing, dressing, meal preparation, housekeeping and running personal errands. The following are the senior services we provide: Homemaker Services: Provides assistance with housekeeping chores such as sweeping and cleaning the house, laundry, meal preparation, grocery shopping etc. Personal Care Services: Provides assistance with activities of daily living such as bathing, grooming, dressing, meal preparation, feeding and housekeeping chores etc. Respite Care Services: Provides temporary staff to support you for a short time to allow your caregiver some time to rest. Community Living Services: Provides community based residential service in an integrated community setting of your choice as an alternative to nursing home placement. This option allows seniors to share a home with no more than 2 other people. Well trained health care staff and direct support professionals are provided to help you with personal and health care in an independent living arrangement.
